Insurance Plans:
CVOMG accepts most insurance plans. Please call any of our locations for more information.

Forms:
A fee of $25.00 will be collected prior to completion for each form that requires the doctor’s signature. Allow 24-48 hours for disability forms to be completed.

After Hours Care:
If you have a medical emergency, please call 911. If you have a non–emergent question or need, you may call any of our 4 locations and the phone service will contact one of our CVOMG physicians. If none of our physicians are on call you will be connected with a local physician that we trust, to care for you in our absence.

Billing for Services:
You are responsible for supplying all appropriate billing information, including insurance identification, the correct billing address and contact information. Please notify us of any changes in your insurance, phone or address information.

Hospitals:
• CVOMG’s physicians have privileges at 2 major hospitals; Community Regional Medical Center-Fresno and Clovis Community Medical Center.

Should a patient deliver at night or require other emergency care please observe the following to ensure you receive the proper treatment:
1. For any emergency that does not require an ambulance, please go directly to the closest hospital and they will contact the physician on call. If you are less than 20 weeks pregnant, you should go directly to the Emergency Room, 20 weeks gestation or greater you will go directly to labor and delivery. When in doubt please go to the hospital, as the safety of yourself and your baby is of utmost importance.

2. For a minor problem or question that can wait until the next day, you can call us from 8:30 a.m. to 12:00 p.m. and from 1:30 to 5:00 p.m. The office staff will help you with your question or connect you with the physician. Please do not call our physicians in the middle of the night for minor problems such as what to take for a cold or refills for medication. We are happy to answer these questions during normal business hours and want to ensure that our physicians are attending to those who truly have an issue that cannot wait.

3. Before you reach the seventh month of pregnancy it would be highly advisable to pre-admit. All the forms to be filled out will be in the main admitting office at the hospital you want to deliver at. When you go into labor, all your paperwork will be done. Upon arrival at the hospital, enter through emergency admitting to sign one form and then you are on your way to labor and delivery.

4. Although you have your doctor, your baby also needs a doctor. By the seventh month of pregnancy all patients must furnish us the name of the physician who will care for the baby after birth. It is your responsibility to arrange for a doctor for your baby. If there are complications and a specialist is needed for your baby, the doctor will let you know and will make the necessary arrangements.
